The word seems to be a rare diminutive form of pile1, equally uncommon in Continental French. As such it would be a vernacular reconstruction of the Latin diminutive pilula. Alternatively, the term could be interpreted as an unusual spelling variant of pelote (which is also used with the sense ‘(pharm.) pellet, medicinal ingredients compressed into a bolus or pill’).
